I here by authorize Chompie's or it's designees to contact, obtain, and verify the accuracy of information contained in this application from all previous employers, educational institutions, and references. I also hereby release from liability, Chompie's and its representatives, for seeking, gathering and using such information to make employment decisions and all other persons or organizations for providing such information.

I understand that any misrepresentation or material omission made by me on this application will be sufficient cause for cancellation of this application or immediate termination of employment if I am employed whenever it may be discovered.

If I am employed, I acknowledge that there is no specified length of employment and that this application does not constitute an agreement or contract for employment. Accordingly, either the employer or I can terminate the relationship at will, with or without cause at any time, so long as there is no violation of state or federal law.

I understand that it is the policy of this organization not to refuse to hire or otherwise discriminate against a qualified individual with a disability because of that person's need for reasonable accommodation as required by the ADA.

I also understand that if I am employed, I will be required to provide satisfactory proof of identity and legal work authorization within three days of being hired. Failure to submit such proof within the required time shall result in immediate termination of employment.

CONSENT TO PROCUREMENT OF CONSUMER CREDIT REPORT
I understand that, as a condition of my consideration for employment with Chompie's, or as a condition of my continued employment with Chompie's, Chompie's may obtain a consumer report that includes, but is not limited to, my creditworthiness or similar characteristics, employment and education verifications, social security verification, criminal and civil history, personal interviews, DMV records, any other public records and any other information bearing on my credit standing, credit capacity, character, general reputation, personal characteristics and trustworthiness. I hereby authorize and consent to Chompie's procurement of such a report. I understand that, pursuant to the federal Fair Credit Reporting Act, Chompie's will provide me with a copy of any such report if the information contained in such report is, in any way, to be used in making an adverse decision regarding my fitness for employment with Chompie's. I further understand that I may request a copy of such report, along with the name and address of the reporting agency that produced the report.

I understand that a complete application is required for consideration of employment.
